This is a “Fan 1 Performance/Stuck off” trouble code readout.
While on a medical trip in Washington DC, check engine light came on after a short drive (about 6miles) on a hot day. Warning indicates dealer visit required so I immediately contacted nearest dealer about 20 miles away and got scheduled. I drove the vehicle shortly after and check engine light went away but came back on after restarting.
Needed to pickup meds a day later so had to drive it again this time check engine light stayed on. On return trip, I noticed AC starts blowing warm air when idling (i-stop off) but starts cooling when vehicle is running. That means radiator fan is not at all running but there’s airflow to radiator and engine when driven. I suspect either a faulty temperature sensor or bad relay for electric fan.
“Extreme temperature warning”came on upon arrival at house so I decided to contact roadside assistance and get it towed. This is “Free service for new vehicles” just so everyone on here can be informed.
It’s now 3 days since vehicle was dropped off at dealer but no further response from dealer yet.
Also, we forced the dealer to give us a loaner car which they said none is available but we contacted Mazda customer service and they got in touch with dealer to provide us a loaner car.
Seems it’s always a fight in situations like this.
I will update this as to progress of repairs.

Update; got the car back after almost two weeks, 1 week waiting for part
Electric fan assembly, p/n PX28-15-02SD was replaced. Warranty covered.
I was curious what the part cost so I looked around; dealer price is $1165 but saw online
parts anywhere from $650 to around $8xx.
I hope it doesn’t fail as temps around here averages high 90s.
